    If I were you I would lower the weight (wouldn't be surprised if 135 is a good place to start) and focus on your form. You aren't performing the squat as detailed in the book. The biggest thing I see is that you aren't pushing your knees out far enough which would allow you to get to depth. As far as programming I would restart with linear progression. As someone already said video as may of your squat work sets as possible.

    I'm pretty sure it's horrible flexibility in my hamstrings.

    Based on Chris' advice over on the programming board, I brought the weight down to 225. Below is my 4th set at 225. I am still learning the iPhone and screwed up the earlier recordings. I think I still might not be hitting depth.

    Almost there, another inch or two down and you'll have it. It's very close but you still won't be getting the full advantage of the hamstring and ass stretch reflex.

    I will have to change my stance since what you saw is as low as I can go without rounding my back.

    Your bar seems still too high on your traps, as well as a few more inches depth. You seem to have hit a mental block in the depth, strength is there, possibly (can not see) that your stance is a little narrow.
