I have several issues with my right shoulder and it can have an affect on the bar in the squat, the bar at times will be slightly uneven.

Let me see if I can get the explanation tonight. I've addressed it in the past, and given up on it when I just wanted to squat.
Maybe you're not doing the right mobility work then to target the issue? Clearly there is a mobility issue, as one elbow is able to be higher up than the other. I'm just not smart enough to know what's causing it.

Well, as I have already said that it is also related to a stability issue, so that's not entirely correct. One elbow being up higher than the other does not have to mean that one elbow/shoulder has more mobility than the other. My body could be forcing one side to be lower to compensate.

As I said, I'll try to ask again tonight.

Ok, just got back from the gym. I can't activate my left QL, so instead I use my right lat to stabilize and it causes me to somewhat crunch down on the right side, thus causing the bar to become uneven, and my elbows follow the bar.

Not saying this is your issue. Just keep in mind that there may be more to it than you think.

    Thanks for the replies

I tried squatting yesterday with this grip and it didn't hurt. Now I'm thinking the pain might just have been part of my other aches and pains which have faded/disappeared in my week off.

Yep I can feel my chest being stretched when I do these. I'll look into and try those stretches you mention, thanks very much

To be honest I can't work out which side is the affected side. I guess it's my right elbow raising behind me?

Any progress? I actually took a video from behind last night to demonstrate what I've got going on.

Did anybody with the same problem identify the root cause?

I have the same issue and I used to get pain in the shoulder from heavy squats.

Moreover, my left foot is pointing outwards a bit more than the right and I'm sure it is somehow related.
