How concerned should you be? I don't know. I probably would not get concerned, I would just fix it. Here is what you should do.
Stop arching your back so much at the top. You are overextending your spine. This is not good. Stand up normally, get tight, and then don't let the spine move. Note, that does not mean arch your back more.
Lean over more. Trust it. You are going into the squat with too vertical of a back angle. This is causing your knees to go forward.
Drive your knees out more. This will also help with the forward knee travel and get you a little extra depth which you need.
