You are on your toes a good bit. Push your knees out a little more and sit back so that you are in balance. Think "heels." You need to be in the middle of your foot. Keep your chin down, your knees out, and drive your butt up on the ascent.

Thank you for your time. The heels cue is really helpful. I've been struggling with my form for longer than I care to admit, and thinking "heels" seems to make it easier to keep my knees out, and to make the bar go up faster as well.
