You are rushing your set up and each re-set. Follow the five-step process outlined on here. Get your back into full extension and then squeeze the bar up. Don't just jerk it up. Pull back a little.
Just hit 295 today and I wanted to iron out any problems with my form before reaching 315. Im really sorry about the angle and the music, I did my best. Also I dont know why the quality is so bad, if that's a problem I can reupload the video. Thanks in advance.

Your lumbar spine is flexed. You're pulling with too much back... fix that before you hurt your back. There are other things like the bar is going forward and all that, but you really need to fix the spine/flexibility first. What's helped me is stretching out my hamstrings. Do some RDL's, learn what it feels like to have a flat back.
