Someone look at my squat?
Are hips moving forward rather than up?? How do I correct this??

It works now, but it's not a great video for a form check. Get one with the camera more at hip height and located to the rear 45 degree position. That being said, you are not nearly tight enough. You need to get your back into thoracic extension and don't have your head so far down (read this). You aren't staying on your feet - your toes keep coming up and they're kind of rolling around. Part of this is probably because you're squatting on a mat on carpet, part is because you are barefoot, but the bigger part is because you're not following the Master Cue for the squat. Don't let your knees bounce forward.
Fix these things, post another form check from a better angle, and we can go from there.

I am going to get shoes this month. This set felt way better and easier then last time. Rep 3, I felt my hips move forward but then corrected it on 4 and 5. Also noticed after I uploaded, I'm still looking down to far

You've got a little more work to do on your filming (camera's still too low and not quite far enough behind you).
It looks like your stance is much too wide. Bring it in a foot's width on each side. Pull your rack a foot or two away from the wall, I think that'll help you get under the bar before you unrack and make getting tighter easier. You've got a little more work to do on your upper back (see how your wrists are bent under the bar a little?), but it's better than in the first video. In general, you can get tighter still.
