What is the recommended fix for this? Does it have to do with toe flair, foot spacing, or something entirely different?

What kind of shoes are you wearing?

And can you push the knees out properly over the feet at a lighter weight than what you are attempting now, or are you never able to do so?

This used to happen to me. For me it was that my feet were too close together and too straight. Try widening them out and putting your feet at a 30-40 degree angle.

the op hasnt answered his thread so im gonna steal it lol, i can keep my knees out till about 120 and they start caving there, even the light weight reps feel difficult when i focus on shoving my knees out hard, when i get to my work sets it just seems impossible

If you care about fixing this problem, then you should stop lifting weights that are heavy enough to compromise your form. If your knees start caving in at 120 and the reps feel difficult when doing it correctly, you might want to back up to that weight.

I had this problem when someone told me to keep my back straight. I was off balance so my knees kept on caving in. Assuming you're doing everything else right, you sound be leaning in at the hips about 30 degrees do the bar is over the middle of your foot.
