I am squeezing my chest up as high as I can but my back still doesn't look/feel correctly set (my knees are out.) I'll fix my neck next time, didn't realize I was doing that (stupid mirrors.)

1) You should be looking at a spot 8-10' in front of you on the floor. Your neck is reamining overextended at the top of the rep, which is bad. You should be standing normally at the top.
2) You never get your back into extension at the beginning of each rep, and you lose control of at least some of whatever back extension you have on each rep. The only time your back is in extension is at the top. Squeeze up harder before you pull. Take a deeper breath and squeeze down on it harder. Some people (me) find it easier to hold the breath, then squeeze the back into position. In either case, you're doing a poor job of it, and it's the biggest problem with this DL.
3) You're trying to stand up immediately off the floor. The initial movement of a DL is a knee extension. You should stay out over the bar with a relatively consistent back angle for the first 2/3-3/4 of the way up the shin.
