You picked up on most of them. Remember to drive with your hips all the way to the top. That will help a little with the sticking point in the middle. Otherwise, things look reasonably well sorted.
I am hoping to get feedback on my squats. It has been a while since I've had someone look at my form and I want to make sure I am on the right track.

The things I see I need to imrpove are:
1. look down, not straight ahead
2. keep the left elbow up, it looks like it drops in the hole
3. stop playing around with my grip mid set
