I've only been there once, but Old School Iron Gym in Cleveland, OH is pretty no-nonsense. Plenty of racks, bars, and plates.
I am still training a a commercial large chain gym. Not the best environment for serious training. A lot of things that go on there irritate the #%*? out of me. That's why I just joined a simple black iron gym. World Gym in Babylon, NY

Hoffman's Barbell in Wheatfield Indiana. Power racks, monolifts, competition and specialty barbells, strongman implements, and lots of strong lifters. Not really set up for any kind of olympic weightlifting, but if you're in northwest indiana and are serious about lifting you should check it out.
Last year I stumbled across the Royal Oak gym in the metro Detroit area. Several racks, good bars, cheap rates, and a strong clientele.
