Question Tweaking my back too often

I have tweaked my back four times in the last five months.

    Age 37
    Height 5' 8"
    Current Weight 173

2015 - started training as per the starting strength program. I don't have logs from those sessions.
But I stopped training when my back tweaked doing the squat. I thought I would get back to it, once I felt better.
    But that didn't happen.

2017 - started training with the SS program. tweaked my back after 5 months. I started with with an empty bar on all four lifts.

2018 - Found a SS coach relatively near by, signed up for group training.

    04/21 - First session.
    Starting Weight 165.5
    Squat -125 lb
    Press - 55 lb
    Deadlift - 155 lb

    ...

    05/18
    Squat - 185
    Press - 55 (reset)
    Deadlift - 220 - Back tweak, reset to 135.

    ...

    06/27
    Squat - 230
    Bench - 127.5
    Deadlift - 225

Had to take two weeks off, and had to reset

07/10 (first session after two week break)
    Squat - 210
    Press - 70
    Deadlift - 205

    ...

    07/20
Squat - 215 - Back tweak, after a couple of more session attempts of 215, reset to 195
    Bench - 120
    Deadlift - tried 135 x3

    ...

    08/24
    Squat - 235 - Back tweak
    Bench - 130
Deadlift - 250 (Could not complete)

    08/27
Squat - 235 - Even though I had tweaked my back, the previous session. I was able to complete the session.
    Press - 85
    Deadlift - 250 (Could do only 3)

    ...

    09/06
    Squat - 247.5
    Bench - 135
    Deadlift - 260

09/08 - Last training session at the SS gym
    Squat - 185
    Press - 87.5
    Rows - 135 4 X 4

I had to go back to a commerical gym, also I changed program for Squat and Deadlift to the GPP 6 day program from BBM.

    10/08
    Squat - 220 X 4 X 4 @ RPE 8 - Tweaked back in the last rep of the second session.
    Press - 85
Chinups (7 mins, As many reps as possible)

Any changes I should make in my programming so that this doesn't happen so often?

Video of the lifts is going to be the way to determine what’s going wrong.

It's not your programming, it's your technique. Also, what do you mean by a "tweak"? Is it mildly irritating or debilitating pain? Sometimes you have to train through certain discomforts.

A back that is strong is much less susceptible to injury. You, sir, are not strong. Not even a little bit. Your back has hurt for > 3 years. Your back hurt when you did nothing and just "gave it time to get better". Your log here has 10 training sessions since April. Is this every one of your training sessions in this time? If not, why not put all of your training in here rather than quick snapshots that are weeks, sometimes, apart?

"debilitating pain" - But getting out of bed the next day is extremely painful.

    starting strength coach development program
Well, painful or just stiff? Every day after a workout, I am very stiff getting it of bed.

And take last part that as you will.
