When judging shoulder width, would the heels be "touching" the outside of the shoulder (if they were on same level) or is the heel directly under the shoulder? It looks to be a difference of a couple of inches.

Nevermind, I reread article and the imaginary plumb line from the ac joint visualization answers my question.
