It’s certainly less common than in smaller joints (great toe, in particular), but gout can show up in just about any joint, either singly or with multiple joints involved.

This was one of the considerations I had when the OP talked about his symptoms. It does seem more gouty than psoriatic arthritis, especially with the clean radiographs.
