    Default Lateral Extra-Articular Tenodesis

    G'day all concerned,

    My wife has a friend in her 30s who has had her second ACL repair. I believe the first one failed due to her falling over in the bathroom.

    She's been out of action for a while but had her surgery. She mentioned something about them cutting her IT band to lengthen it in the surgery and a lek surgery. I think she may have meant a LEAT procedure.

    I'm looking at training her as soon as she has the confidence/permission to begin. Currently still in a brace and swollen but she's got 120 degrees of motion on that knee.

    Are there any issues with this sort of thing that I should be aware of when the time comes or has anyone dealt with this before that has some advice on where to begin?

    How long post-op is she? Can she perform a bodyweight-only squat right now?

    Maybe 6 weeks. She is still on crutches and in a brace. She said her surgeon said not yet on squats.

    My general plan is as soon as she is able and willing, I'll start wherever I need to. If that's bodyweight good, if it's a box then so be it.

    I was more concerned with the rerouting of her IT band, given it is something I had not heard of.

    Ask her for the surgeon's actual report. She doesn't know what she's talking about.

    Just to make sure, you are asking about having someone who just had an ACL repair and LET procedure 6 weeks ago and whether you can squat them right now?

    Not so much right now, more in the new year at this stage which I'm hopeful will be fine. I had just not heard of a LET procedure before and wasn't sure if that requires any long term adjustments.

    If you say I can squat someone who's had this done 6 weeks ago then I'll have more courage to try it but otherwise I think she is happier holding off for now and just doing physio rehab. She is still not walking unassisted and been on crutches for a long time waiting for surgery.

    She still hasn't given me a report yet.

    Okay, thank God. I am a very aggressive therapist, and even for my liking, my initial read was that this was egregiously depraved. No, she should not squat right now. A few more weeks goes by, and that might be an option, but not at six weeks. You are perfectly capable, and I am strongly recommending, that she do whatever unilateral training on the uninvolved side, and any upper body lifts that do not require weight bearing through the surgical side. The LET procedure is very good for outcomes, especially in preventing rotation around the knee after the surgery. It is becoming more common as a secondary reinforcement procedure for augmenting an ACL repair.

    If you’d like, we can connect outside of the forum, and I’d be happy to assist you with this case. The two of us could do more than a physio can.
