    This is probably a dumb question, but on the plus side it should be quick to answer.
    In Starting Strength, the description of the very first phase of training states that the deadlift should be done every workout until the freshness has worn off. Does this mean when you miss the fifth rep?

    No. When it gets "heavier" than it was at first.
