I've usually seen it used in reference to being able to recruit more muscle fibers at once. Not if that's BS or not (I was under the impression that it's not), but seems like those folks were connecting it largely to the really quick early strength improvements that come even before muscle size increases.

Because, of course, Science has proven that you don't gain any muscular bodyweight for at least 6 months, because, of course, the first adaptations are "neuromuscular" improvements. This is a Scientific Fact.

What is going on when an advanced lifter gains strength but doesn't gain any body weight?

How much strength? Has this been your personal experience, or have just read about it?

he might have increased muscle/fat ratio.

might have perfected or tweaked technique for those particular loads ... that might be one definition of the "neuro-muscular improvement" term that keeps being thrown around.

But in order to do this, he'd have to have gained muscle while losing fat. I thought this was only possible for novices.

Just throwing out an example of what Fulcrum was mentioning about it meaning different things to different people. It's how a coach I previously worked with (and a few others folks I know) used it. I'd never really thought too much about it. I think he was mostly using it in reference to beginners only. Happy to be corrected, I'm here to learn and attempt to contribute where I can.
