Wanted to update the old thread, but it's locked.

Called five local shoe repair areas: $120, $89.95, $70, fourth one didn't even give me a quote. Just said "I'd have to see it to even know if I could do it." Fifth one didn't pick up.

I appreciate quality workmanship, but jeez, this is frustrating.

You can damn near do this yourself if you can find some dense rubber for the shim. Contact cement is the glue.

The only required criteria seem to be
    1) stiff enough to provide support
    2) not so stiff that the toe and heel are bound together via the shim

I'll give it a shot and report back.

Horse trailer mat will work. Just clean it with volatile solvent before you glue it.

After some adhesion tests, I got a good combination of process inputs. There is some nuance I did not anticipate, so I'm glad I did the adhesion tests. Total cost was about $15 (Naphtha and contact cement from Home Depot).

If anyone wants a full write up, let me know.
