When instruments at a generation facility freeze off and shut the whole place down, that type of thing trickles down to the level of an instrument tech saying something like “hey we should probably heat trace these impulse lines so they don’t freeze when it gets real cold”, and management saying something like “yeah, that would be nice but we don’t have the money for that right now.” Couple that with operators who have no experience of what to do when shit freezes and you’ve got a disaster.
Having been involved with electrical reliability in an industrial plant setting for the last ten years, I’ve seen it first hand, repeatedly. Except we have plenty of experience fixing shit when it freezes.
