In our clinic (in Germany) we ran a weight loss program for kids AND their families.
On the first day we had an "orientation session" where the kids, their parents and their siblings had to attend.
When I saw those families I was shocked:
some of them barely fit through the door.
Only in rare cases do you find fat kids, it's fat families that are the problem.

Furthermore you need lots of education, knowledge, societal support and training (in the sense of making long term plans, adjusting the plan, self monitoring, etc.).
Surgery alone? No effect in the long run.
Without changing eating habits nothing would change.
