My Dad has pinched nerves in back, losing mobility to walk. Massive stomach, doesn't exercise and weak frame.
Not asking for a diagnosis/treatment, but he sees zero connection to physical activity or lack thereof. I liken it to a tree bent over from too much snow on the branches.

He's prematurely 79.
Your dad will always be the hardest guy to try and get to listen to your advice on anything. He still remembers when you were a helpless, dumb little baby that kept crying and shitting itself, and when it comes to advice he probably feels like he is taking advice from that same baby.

All you can do is give him the info. It’s up to him to decide if he wants to salvage himself. You may be able to help him.
It will be an uphill battle but it may be worthwhile to not give up just yet.
Getting physically strong is certainly possible for him and if he can, it will go a long way in slowing the progression of entropy and frailty and even reversing it to some extent.
His doctor probably won’t be of much help but you might.
