Would one need to attend SS Seminar Level 1 before he/she can attend Seminar Level 2?

No. The Level 1 is not a prerequisite for going to a Level 2 Seminar. Did you read this?

Not many. Those that do probably do not have a a great interest in identifying themselves as Starting Strength Coaches. A competent coach will identify people whose shoulders cannot accommodate the low bar position and move to the next best version of the squat - high bar. However, becoming a Starting Strength Coach usually selects for people that agree with Rip's arguments and analysis regarding training. Folks change their minds over time, but most who went to the trouble of earning the certificate tend to prefer the low bar squat.
