I would suggest fixing your gaze about 12' in front of you. Your neck is too bent especially in the lowest position.
Pretty good. As the last step before the pull, you'll want to take all the slack out of the system. Listen for the bar to click, then lift. This will become more important as the weight on the bar goes up.
Stand straight up at the top more consistently, and lower the bar by shooting your hips back so that you clear your knees.
Creama is right that your neck is craned slightly. Also try to avoid rolling the bar forward immediately before the pull. That happens, albeit slightly, throughout the set.
