    Stance is a little wide and you are sliding forward at the bottom and lifting your chest on the way up. Narrow your stance just a touch, lean over more, and drive your hips up all the way to the top. Don't lift your chest early. Stay with the hips. Leaning over more will help with the slide forward at the bottom, too, most likely.

    My stance felt and looked narrower, but honestly I can't say that it looks much more so to me in the video. Was trying to stay more with the hip drive and bend over more.

    This was probably pretty close to a rep max, so form break-down more likely.

    My earlier comments still apply. Your wide stance makes getting your knees out harder, which interferes with depth and a few other things.
