I've seen a lot worse. From the front, I though you might be rounding a bit, but it doesn't look that way from the side. Keep adding weight. Fucking around less while holding on to the bar will only help, too.
After noticing I rolled the bar forward before starting a rep, (following being told that I was) and actually watching my own video I posted... I established I was setting up with the bar BEHIND my midfoot.
