You have a little forward knee travel at the bottom. Not a ton, but some. Try to get the knees out and in place early in the descent, then just sit back.

Stop moving your head and eye gaze so much. Neutral neck, eye gaze down.
Things were a little wobbly, but overall fine. Especially if this is a 5rm.
