    I see: bad depth and some lower back rounding on the last rep. Do I have it all?

    Your depth is fine. Hell, you've got a box there to help with that. You need to stay tighter at the bottom. You noticed that your back gets soft on the last rep. It happens on a few of the others, too. You shoot your but back instead of up and then round the back a little on the ascent. Small stuff.

    So just focus on squeezing my back tight out of the bottom as hard as I can? Interestingly, I dont have this back rounding problem when I dont wear a belt.

    I think you are relaxing on the box, so you may want to consider ditching it. Concentrate on maintaining maximum tension throughout, not just in your back. drive your shoulders aggressively against the bar on the ascent. See if that helps.
