I've been told I lean forward to much when squatting, and I think it's the reason I almost lost it on the 4th rep on this video:

I want to get my form perfect, criticism greatly appreciated.

You're squatting solely with your knees...

    Start using your hips.

I agree actually, I feel my knees working a lot. During my warm up sets I think about all the things I need to be doing and that often results in leaving some things out.

Will bearing the hip drive in mind fix my forward lean?

There is this book that will help you out. You should try reading it.

I don't think you are leanig forward enough. Your knees travel too far forward because you are trying to stay too upright. Sit back more with your butt and don't be affraid to lean forward as much as needed for your torso/femur proportions. Someone with longer femurs in relation to torso will need to lean forward more than someone with a long torso and short femurs. The squat is a total body lift, not a quad exercise.

I've read the book thank you.

Is hip drive the only thing I am missing out on here?

I was gonna say you must of just looked at the pictures, but I doubt you even did that. If you had, you'd know that your squat is all sorts of wrong.
