    I suspect your bar placement is too high, make sure it is directly below the spine of your scapula.

    These are all very high. If you've been squatting like this for a while you're going to need to take some weight off to hit depth. This might be 205, might be 185, might be 135.

    When you squat: LOOK DOWN at spot on the floor about 5' in front of you (if you can switch the hooks to the other side of the rack and face away from the mirror that may help a lot. When you start the movement: Point your nipples at the floor and shove your knees OUT hard, after you hit proper depth drive your butt up.

    One thing I should note, I am currently doing High bar squats. My shoulder flexibility is bad and I end up in to much elbow pain when attempting to do low bar squats. It's a work in progress.

    OK, then do so. Don't lean over so much and get your knees further forward.

    Speaking of knees, they are going forward; they need to go out and stay there otherwise depth's not going to happen.
