The bar assymetries do not seem significant. I wouldn't worry about this too much. The main thing I see is that you are moving from your hips (trying to emphasize horizontal back angle no doubt) but really your knees and hips should unlock together.

Please could I get a check on my squat form? I attended the seminar at WF in September but just want to make sure I've got it right on my own before increasing the weight.
The main problem I can see is that the bar appears to be tilting to the left at the bottom, but I'm not sure why that happened or how to fix it! Any other criticism would be greatly appreciated.

That lower back, what looks like relaxation, when reaching the bottom, is that okay?
I ask because I do the same thing (and noticed a lot of others) to some extent. Even when trying to keep tight and able to tilt the pelvis as in the book.
I also noticed the issue with your knees/hips. Do what Brent said, and make sure you are not holding your knees back. Let them go forward, they will stop when they need to and you will be fine.
