    1) At the top, you're leaning forward at the hips. Don't do this. Stand straight up. You lean forward slightly at the ankle to keep the bar over the midfoot at the top.
    2) Bend over more. Right from the start you need to think "bend over." Your knees are travelling a bit too far forward, and you're not bent over enough at the bottom.
    3) I can't really comment on stance width / toes, because I can't see them, but it appears toes are not turned out far enough (this is a bad camera angle) - you need to read the post at the top of this forum and do your best to video your form checks in accordance with the suggestions there. It helps us to better help you.
    4) Drop your elbows a little bit, and try to work your hands closer together.
    5) You're doing a good job driving your hips up out of the bottom. Keep it up.

    YOu understand, having read and understood that book, that if I thought depth was an issue I would have said something about it - somewhere up around #1. Right?
