I'm not dead, I haven't quit, I just did something really, really stupid, which was trying to squat 225 for the first time, but leaving a plate off the right side. I wound up hurting everything.
Anyway, I think I'm recovered, hopefully I'm a little smarter and I'm back after a long layoff. I bought a safety squat bar because I thought it would help not tear up my shoulders.

I've done that a few times over the.years. Your SB squats nmbers look good.
Yep, been there, done that. Thankfully I've noticed on a squat just as I unrack the bar and on the deadlift just as I start my pull and notice that one side comes off the floor and the other doesn't. Soon you'll be pretty compulsive about verifying, especially when you get into grind territory. And I think that Mr Bingley said it succinctly:
Take it slow. You've got a lifetime ahead of you.
