    It didn't look like your back was ever set before you lifted.
    You have survived, so it wasn't that bad.

    Take also a closer look to the downward path of the bar, this should be vertical.

    Please submit sets of 5 for form checks. And yes, you're back was not set properly. Work on really squeezing your chest up and getting your back flat.
