You're shallow. Well, maybe not your personality, but your squats sure are.
Bring your heels in just a little and stay turned out. Then drive your knees out so they stay over your feet - this will give you a hole to drop into. You're also letting your knees get too far forward. They really shouldn't get much past your toes and they should get there by the time you're halfway down and stay there until you're halfway back up. Also, don't drop your head so much.
