Not bad, though there are a couple of things that can be dialed in. First, you're letting your back (especially your upper back) move around during the squat. Get your upper back set in extension before you start the descent rather than setting it as you move. Read this article, it helps a lot!
You are also shallowish and are getting into your toes. Part of this is due to the change in balance as a result of your upper back moving. Part of it is your knees. Drive them out harder and then stop them from moving forward after you're halfway down. You may benefit from using a TUBOW.

In this instance, I was training at the downtown Seattle YMCA. Depending on my work schedule, I train there or my home garage gym.
You've got a number of coaches in your area. If you can get a session with one of them, they should be able to get you dialed in really quick.
