There is some movement of your knees near the bottom. Think about pushing them out hard as you near the bottom and holding them out. If you cut about an inch off of your depth it should also help with that.

About 2 inches too deep.
Your knees should be in their final horizontal position at about a half squat. Think "knees forward quick then sit ass down and back".

Too deep and feet are a bit too wide. Bring them in an inch on either side.

I'll post a re-check this weekend and see if I can correct the issue

The third set though is too fast on the descent.
