1) Get the rubber mat further forward. Your feet are halfway off the mat when you're unracking!
2) Your stance needs to be an inch wider at the heels and the toes need to be pointed out at 30 degrees (they are around 10 degrees right now).
3) Look at the spot where the wall meets the floor
4) It looks like you are cranking your elbows up. While this is mentioned in the book, a clarification has been posted about over-doing this cue. Read the "Upper thoracic rounding" article for more details.
