Hello,
I think I am getting towards the end of LP. I want to try to fix all of my form problems before transitioning to a more intermediate program

For squat, work on decreasing your setup time. It must be burning up a lot of energy.
Stop doing that hip flexion-extension-flexion thing you do before even initiating your rep.
