Knees are caving, here's the "why" that is important to correct: Knees Out/Toes Out in the Squat | Mark Rippetoe

Two additional comments; 1) It took you 19-20 seconds to begin your first rep after removing the weight from the rack; way to long. 2) Lifting shoes?

I assume you mean "verified user?" Good idea, make sure to read the sticky on posting videos.

    Setup:
    - Stop waiting so long before you squat. Wasting energy.
    - You stance looks a bit too narrow (look at your knees on the way down).

    Movement:
    - Your knees cave in on the ascent. I believe it's because of your narrow stance first, and possibly your toes pointed out too much.

    Fixes:
    - My suggestion is to widen your stance a bit and think to shove the knees out HARD.

"KNEES OUT" should be in your mind the whole time.
