Looks like you're lifting your chest early on the way up.
It looks like you're burying these 1-2" too deep at the bottom, just go down until your hip crease falls below the top of your patella. And connected to lifting your chest, take a look at your head and neck during these. You start out decent, but reps 3-5 you're leading with the top of your head. Keep your neck neutral and in the same position during your lift. Your eyes are staying at the point on the floor that they should be, but your neck and head are roaming.
