Knee slide, have an idea what this is

HERE is the video.
As you can see, the first rep is good, and that is when I think I am
a.) not breaking at the hips and knees at the same time, thus causing the slide
b.) losing tightness at the bottom but not reaching back with the hips.
I am pretty sure it's A, and need to get back in the gym to diagnose but wanted to know if you notice anything else glaring...

A.) that's right. You do a better job of this in rep 1, but your knees start lagging more and more as the set goes on.
A leads to B. Late knee movement prevents the hamstring tightness that you need at the bottom of the squat.
Solve the knees and hips breaking at the same time. My trick to do this is to have you jump. Don't think about it, but jump. A nice deep jump as if zombies are coming after you. Now, do it again and think about when your hips and knees and chest broke in the jump. Do the same thing when you start the squat. See if it helps.
