    It looks like you're rounding your back in order to hit depth, which I believe is slightly excessive. Work on holding holding your lumbar in extension, and stay tight at the bottom. Also, the chuck taylors and the carpeting are harming the mid foot balance; see your feet wiggling around? Lifting shoes and a hard surface will help a lot if you can swing it.

    Welcome to the board! Your stance is too wide, get your feet closer to your armpits and get your toes pointed out 30 degrees. As it stands right now with your stance too wide your knees aren’t tracking over your feet. So fix your stance and shove your knees out hard on the descent. I’d wager a guess you could also get your grip a little closer and your upper back a little tighter (use the Horn stretch if you have tight shoulders).
