I would widen the stance a little. Your hips are going back before your knees causing your knees to slide forward throughout the reps. Your knees and hips need to bend at the same time. Think about sitting back and not letting your knees come forward

Hi Ryan, I've been slowly widening the stance bit by bit while the weight has been getting heavier during the last months. I'll try an even wider stance next time and work on getting the knee+hip bend synced up.
