You're set up too close to the bar. When you're standing in the set up, crane your neck to the side to get an idea if the bar is bisecting your feet. Looking from the front down is a poor angle.
You're not setting your chest on any of these. Before you pick up the bar, lift your chest and look at one spot on the floor about 7-10ft in front of you.
Stand up fully at the top and show off your chest.
