Hello dear coaches, I would appreciate your assistance with my squat form

    YouTube

    Some points to mention:
I feel like the bar wants to roll up my back, and when it wants to do so I bend my wrists into extension in order to prevent it from rolling up.

    Height: 185cm (6,1)
    Weight: 95kg (209lbs)
    Age: 16
Weight on the bar: 140kg (308lbs)

Looks like a good hard set of 5 to me. The bar did not appear to move. Start working on decreasing the amount of time you spend in between reps breathing.

The bar is moving forward and rolling up, mainly visually visible on the third rep, how should this problem be fixed?

I don't see the bar rolling up on your back. The reps are hard and on the way up you drive your elbows up a bit to "help" with the lift, so that rotation is what you might be seeing - it rotates the bar slightly when you do this. This isn't the end of the world, but ideally you won't drive up with your elbows. So, practice staying in your hips to generate force on the hard reps.
