Pretty good. Unrack with straight wrists - the bar should be resting on the bones of your forearm, not set back in your hand
I did a rear shot, and 2 different rear quarter shots due to the rack uprights getting in the way. Because of my setup and because my girls were squatting off the front rack I couldn't do a front quarter angle

Good question
The head should stay in line with the back. Sometimes you'll see people thrusting their head forward - especially people who are thinking silly things like putting their head through a window when it's not about the head at all - but the ones I looked at here didn't look like this was going on. At the lockout, the torso is angled forward and head along with it.
