    The bar is placed too high on the back, that's why its rolling forward throughout the set

    Looks like you started with the bar in the right place and it slowly crept up as the set went on. May have to widen the grip a bit and focus on keeping it in place. Drive your hips the whole way up and keep your chest down. Use spotter arms
