    Please read the sticky for form check video angles.

    1) Wear a cotton shirt to assure the bar stays in place on your back
    2) Slow and controlled on the way down. You are going too fast and losing tightness
    3) Sit back more and shove your knees out harder

    In addition to what Ryan said, you're very loose in your upper back at the start of each rep. Before unracking, get tight under the bar and think 'big chest' to get your thoracic spine into correct extension. This will help to create the shelf the bar rests on during the squat. It's very hard to get tight after you're out of the rack with the bar on your back. This tightness needs to be maintained throughout the rep.
