Below I have also included a form video from a couple of days ago ...hopefully ive improved and doing it right now?

It looks like the bar was in the right place on your back when you unracked it so you need to keep it there throughout the set(it rolled forward). Don't let your thoracic spine relax during the set. You may need to think about actively pulling your elbows down a bit while keeping your chest up(in relation to the bar not the floor). Your low back is also not staying extended. A good belt may help
