I think the best place to start is to be more meticulous with your technique - you are too fast and loose. Dip under the bar and create your proper setup position before you *slowly* stand up with the bar and *slowly* take it out of the rack. Don't rush getting your stance and make sure you are super tight in your abs and quads before you start each rep. Keep elbows in and slightly in front of the bar (tight armpits). Don't rush into each rep - get your proper setup, take a breath and get tight before doing each successive rep. Implement all of these things and then let's see how things look.
