Get your eyes out about 10-12 feet in front of you.
Rather than "snapping" your scaps back to set the back, lift your chest up and point your nipples at the wall in front of you as a slow, steady motion.
Then, rather than yanking the bar off the ground, push your feet through the floor and imagine smoothly peeling the plates off the ground.
Just need to set your back properly and quit yanking the bar. Everything smooth and steady.
